Understand what an automated failure means, what evidence is available and why human review can still matter.

What a failure means

An automated test failure means that the scanner detected a condition that did not satisfy an automated accessibility rule.

It does not necessarily mean that one scanner record equals one unique user-facing accessibility barrier. Repeated components can create repeated failures across multiple pages.

Evidence

Evidence may include the affected page, element or selector, the rule that failed and other technical context captured during the scan.

Use this evidence to understand the problem before creating remediation work.

Human judgement

Some automated results require review to understand their real-world effect or to confirm whether the detected condition is genuinely a failure.
